SIGNING AND SENDING YOUR INDIVIDUAL PROVIDER CREDENTIALING RECORD

Enter your username and password. Click on the Application icon. Application Review the information in your application for accuracy and completion. To review information entered into your credentialing record, go to the black navigation bar. Sequentially click on all underlined categories (see below).

To send your application, go to Step 4 and click on the Send Application link.

This action brings you to the Submit Application screen.

On the Submit Application screen select application type from the pull-down menu. Click the radio button of the entity Preference. Click next.

This will bring you to the Document Selection List. Please select all documents.

You will then reach the Billing Summary screen. There is no charge for sending this application. Click the Confirm button.

This action brings you to the Confirm Application Submission screen. Please enter your Authorization Code (digital signature) in the Provider Authorization Code field and click Submit Application.

In the next screen you will have the option to generate an Application Packet. This is a PDF that can be printed or saved to your desktop. This is your sole opportunity to print out a completed copy of your individual provider credentialing record.
