The purpose of this study is to prove advantage and accuracy of this jailed pressure wire technique after post dilatation of stent at high pressure Methods: We conducted a new jailed pressure wire technique using an optical fiber (OptoWire® [Opsens, Quebec]). A pressure wire was advanced in a SB and a conventional wire in the main vessel (MV). Drug eluting stent was deployed in the MV and an additional dilatation was performed in the MV with non-compliant balloon at high pressure while the pressure wire was jailed under the implanted stent. Post value of FFR in the SB was measured under the jailed pressure wire. If the FFR was >0.75 in the SB, no further SB intervention was necessary. The jailed pressure wire was pulled back carefully to the distal tip of the guiding catheter to assess drift. If the FFR was ≤0.75 in the SB, a kissing balloon technique (KBT) is performed. Results: 37 lesions (35 patients) were enrolled. Device exchange was 5.9±1.8 times with pressure wire in the SB. Post balloon inflation pressure after stenting was 19.5±3.6 atm. The jailed pressure wire technique successfully performed and FFR in the SB was measured in all cases. All FFR pullback curve of jailed side branch could be obtained without no stress. The pre and post values of FFR in the SB after post balloon inflation were 0.83±0.12 and 0.84±0.13. There were 6 in 37 (16%) SB that post value of FFR was ≤0.75, then KBT was performed. Final mean fluid filled aortic pressure (Pa) minus mean distal fiber-optic sensor coronary pressure (Pd) was 0.59±0.90 mmHg. Pd/Pa was 0.99±0.0097. None of the 37 lesions (0%) showed pressure drift >3mmHg. Comparing the non-KBT and KBT groups, mean procedural time was 77.0 vs 116.2 min (p=0.0001); mean contrast volume was 86.8 vs 118.3 ml (p=0.037); mean fluoroscopy time was 26.6 vs 44.6 min (p=0.0022); mean radiation dose was 1.7 vs 2.9 Gy respectively (p=0.0024).
