Background: Type 2 Diabetes Mellitus (DM) is chronic metabolic disease that marked by increased blood sugar level (BSL) and HBa1c value. The main factor is bad lifestyle including dietary habit, stress level, and sedentary lifestyle. DM also known as “life long disease,” because no definitive treatment so far and high risk of complication and also increasing of death risk. So they must controlling their BSL and HbA1C value. Method: This research was a descriptive study to identify type 2 DM patients from four PHC in Binjai. Twenty samples were collected from every PHC. Then we sort them by gender, age group, education, after that we will measure their weight, height, and Blood Sugar Level and HbA1c value. Results: The results showed that from 80 patients, the age group 45–65 years were the largest group (61 patients), mostly were females (52 patients or 65%). Patients with BSL of >200 mg/dL were 58 patients or 72,5%. Samples with Hba1c of >8% were 61 patients (76,25%). Samples with normal BMI group were 40 patients (50%). Conclusion: Most of the Type 2 DM patients in PHC of Binjai had BGL >200 mg/dL and Hba1c >8%, thus in bad glycemic control status. It is important to educate diabetic patients to increase their knowledge and self management of BGL.
