1 DEFINICIONES 1.1 BACTERIURIA ASINTOMATICA 1.1.1 Presencia de bacterias en muestra de orina evidenciada por microscopía o cultivo tomada de un paciente sin síntomas de IVU alta o baja. El hallazgo de bacteriuria asintomática debe ser confirmado por 2 muestras consecutivas.
1.2 BACTETIURIA 1.2.1 Presencia de bacterias en muestra de orina evidenciada por microscopía o cultivo.
1.3 BACTERIURIA SIGNIFICATIVA 1.3.1 10*6 UFC/ml en una muestra aislada fresca. En grupos poblacionales especiales los umbrales pueden variar: · Mujeres con IVU sintomática: 10*2 UFC/ml. · Hombres: 10*3 UFC/ml (si el 80% de la muestra en crecimiento está dada por un mismo germen) Punción Suprapubica: 1 UFC/ml
1.4 BACTERIURIA SINTOMATICA 1.4.1 Bacteriuria significativa tomada de un paciente con síntomas de IVU. La muestra única es suficiente.
1.5 IVU BAJA 1.5.1 Evidencia de IVU con síntomas sugestivos de cistitis o uretritis (disuria, pujo o tenesmo vesical sin fiebre, escalosfríos, o dolor lumbar, sin Signos de Respuesta Inflamatoria Sistémica)
1.6 IVU ALTA 1.6.1 Evidencia de IVU con síntomas sugestivos de pielonefritis (lumbalgia, palpación dolorosa en flancos, fiebre o u otro signo de respuesta inflamatoria sistémica).
1.7 PIURIA 1.7.1 10*4 leucocitos (WBC)/ml en una muestra fresca de orina. Niveles altos de WBC pueden encontrarse en mujeres sanas asintomáticas. La piuria está presente en el 96% de los pacientes sintomáticos con bacteriuria 10*5 UFC/ml, pero sólo en 90%, indican inicio de tratamiento empírico. La presencia de IVU a repetición ameritan toma de cultivo. Evaluar siempre la presencia de flujo vaginal, en dado caso evaluar la presencia de una enfermedad de transmisión sexual y vulvovaginitis. IVU alta: Diagnóstico clínico, presencia de puño percusión en fosas renales y fiebre incrementan al probabilidad. Se debe tomar urocultivo para reajustar o confirmar la terapia empírica. Paraclínicos: Parcial de orina tiene baja sensibilidad y especificidad, recordar que la presencia de nitritos no indican IVU. Tienen mayor especificidad la presencia de piuria y gram de orina sin centrifugar positivo. Urocultivo: Bacteriuria significativa se presenta en la paciente con síntomas y 10*6 Unidades Formadoras de Colonia (UFC) tomada de una muestra de orina a la mitad de la micción; o cualquier UFC obtenida de punción suprapúbica. 5.5.2 BACTERIURIA ASINTOMATICA Los pacientes sin síntomas en presencia de bacteriuria no deben recibir tratamiento antibiótico, lo anterior incluyen pacientes con diabetes mellitus, cirrosis biliar primaria, edad > 65 años. El tratamiento aumenta los efectos secundarios sin beneficio clínico. Solo deben recibir tratamiento: 1. Embarazadas, 2. Pacientes que requieren exploracion de la vía urinaria, 3. Pacientes llevados a cirugía mayor. 4. Inmunosuprimido 5.5.3 MUJER EMBARAZADA Ocurre entre el 17% y 20% de las embarazadas. Se asocia con amenaza de parto pretérmino, ruptura prematura de membranas, amenaza de aborto y aumento de la mortalidad en la embarazada. Diagnótico: Punción suprapúbica es el Gold Standard. Si se toma una muestra de orina a la mitad de la micción se debe obtener 10*6 UFC en dos muestras distintas (E 80%). Todas las pacientes deben recibir tratamiento antibiótico. 5.5.4 HOMBRE ADULTO ADULTO JOVEN: Considerar dentro de los diagnósticos diferenciales prostatitis, infección por clamidia, epididimitis. Todo paciente con síntomas urinarios debe tener urocultivo, como corte 10 3 UFC. Considerar IVU alta en todo paciente con fiebre y dolor lumbar, el urocultivo debe guiar la terapia. ADULTO MAYOR: Considerar manifestaciones generales especialmente alteración del estado de conciencia, astenia, anorexia, deterioro funcional. Recuerde las coomorbilidades y factores locales como disminución del tono muscular, hiperplasia prostática. Los gérmenes principales provienen del tracto gastrointestinal. Como corte 10*6 UFC. Considerar IVU alta en todo paciente con fiebre y dolor lumbar, el urocultivo debe guiar la terapia. 5.5.5 PACIENTE CON SONDA

Todo paciente con sonda vesical aumenta el riesgo de IVU entre el 2% y 7% por cada día. A largo plazo todos tienen bacteriuria. No hay evidencia de algún criterio que sugiera IVU en estos pacientes. Sin embargo se debe pensar en iniciar antibiótico por IVU en presencia de: 1. Fiebre 38° C por más de 12 horas, 2. Nuevo dolor suprapúbico o en el flanco, 3. Cambio en las características de la orina, 4. Alteración del estado de conciencia, 5. Ausencia de otro foco infeccioso. No se debe utilizar el parcial de orina como diagnóstico de IVU, algunas referencias recomiendan utilizar el gram de orina centrifugado. 5.5.6 IVU NOSOCOMIAL Presencia de síntomas urinarios y urocultivo positivo en un paciente con más de 48 horas de ingreso hospitalario o dentro de los primeros 7 días del egreso hospitalario. 5.5.7 SITUACIONES ESPECIALES DIABETES MELLITUS: Considere en IVU alta tomar Rx de Abdomen Simple para descartar Pielonefritis Enfisematosa; en el caso de dolor en hipogastrio considere desacartar Cistitis Enfisematosa. HONGOS: El diagnóstico de IVU por hogos debe ser posterior al excluir otros sitios de infección, la candiduria no indica inicio de tratamiento a menos que requiera exploración de la vía urinaria; recuerde el adecuado control de las cifras de glicemia. SEGUIMIENTO AL TRATAMIENTO: En el caso de requerir exploración de la vía urinaria o alguna intervención urológica urgente una vez instaurado el tratamiento antibiótico, realizar un gram de orina sin centrifugar en lugar de urocultivo.
