W e've reported the results of our international survey of contact lens prescribing each year in this journal since 2002, and our analysis for 2007 represents our most w ide-ranging study to date. In this article, w e report on the outcomes of 19,971 contact lens fits performed in 27 countries. As in all previous years, the study w as prospective in that w e asked practitioners to provide information about their first 10 patients prescribed w ith contact lenses after they received paper or electronic survey forms. total number of contact lens practitioners w as few er than this. The precise composition of the practitioner cohort (the proportion of ophthalmologists, optometrists and opticians) necessarily varied by country, but w e chose a randomized selection of those practitioners licensed to fit contact lenses in each case.

Demographics and Background Information In all countries, most patients w ere female (ranging from 56 percent in Greece and Kuw ait to 75 percent in Hong Kong) and the average age at fitting w as 30.4 years (from 26.4 years in Croatia to 35 years in New Zealand). Data in Table 1 suggest that fitting older patients is more commonplace in the developed markets. Australia, Canada, Germany, the Netherlands, the United Kingdom and the United States all have mean fitting ages of about 33 years or older, compared to younger patients in the developing markets of the w orld, particularly in east Asia, eastern Europe and the Middle East (the mean age is under 29 years for Bulgaria, China, Croatia, Hungary, Lithuania, Romania, Russia, Jordan and Kuw ait).

Figure 1 show s the proportion of new fits around the w orld, from China w here only 13 percent of fits w ere described as new , to Portugal w ith 61 percent. In 21 of the 27 surveyed countries, refits w ere more common than new fits.

Soft vs. GP Lenses Soft lenses account for more than 90 percent of fits w orldw ide; they represent 95 percent or more of fits in nine of the 27 countries evaluated and all reported lens fits in China, Kuw ait and Lithuania. This year saw a similar rate to previous years of GP lens prescribing at 9 percent of all fits, including 1 percent for orthokeratology (Table 2). This latter modality w as reported at 4 percent of fits or greater for Australia, Hong Kong, Malaysia and the Netherlands. Interestingly, the reasons for this modality's popularity appear to be different depending on the market. Ortho-k is presumably popular in Hong Kong because of the high prevalence of myopia. The Netherlands is home to a number of important ortho-k innovations, and practitioners in this country have a history of greater GP fitting. Orthokeratology may be popular in Australia because of the significant amount of research in the field conducted in that country.

GP Lenses 2007 saw a continuation of the recent trend for GP lenses to be used for more complex fittings (Table 3), particularly in countries w here the overall use of GP lenses w as low . For example, in the Czech Republic, GP lenses accounted for only 3 percent of all fits and a majority of these w ere toric lenses. In Spain, about one-third of new GP lenses prescribed w ere multifocals. You can see a similar trend for orthokeratology lenses. In Australia and Hong Kong, for example, about half of all GP lenses prescribed w ere for orthokeratology.

The only market w ith a significant fitting rate of 'non-orthokeratology' extended w ear GP lenses w as Norw ay, w here about one in four rigid lens fits w as for extended w ear. This country has a small proportion of GP lens fitting overall and a very high number of extended w ear fits for soft lenses. GP lens planned replacement accounts for about one-third of all fits, although significant variation exists in this proportion among the markets — this value is generally higher in countries w here the number of GP lens fits is relatively small.

Soft Lens Fitting The use of silicone hydrogels for soft lens fitting continues its recent popularity; these materials now account for more than half of soft lens new fits in the Czech Republic, Lithuania, Romania and the United States (Table 4). These findings suggest that these materials have been particularly w ell received in some of the w orld's developing markets. If w e restrict analysis to daily w ear spherical lenses only (on the basis that some markets do not have silicone hydrogel torics or multifocals), 29 percent of new soft lens fits are w ith silicone hydrogels. This is a remarkable figure given that the use of this material for daily w ear w as close to zero in 2004.

Lens Fitting in Hong Kong It's apparent that the major grow th markets for the contact lens industry over the coming decade are in east Asia, particularly in China. This country has the key ingredients for a potentially booming contact lens market: a high prevalence of refractive error, a currently low penetration rate of contact lenses, a huge population and grow ing economic strength. In fact, w ith 600 million people requiring refractive correction in China, achieving a penetration rate for contact lenses of 6 percent of its ametropes (a value typical of a low penetration rate in a w estern European nation, and much low er than that of Japan or the United States) w ould see China comfortably become the largest market for contact lenses in the w orld. As a relatively w ealthy and autonomous region of China, the present situation in Hong Kong represents a microcosm of the future market on the Chinese mainland, and w e have been able to capture prescribing trends in Hong Kong for the first time in 2007. The indicators suggest that GP lenses may not make a significant impact on the Chinese market in the medium term w ith the exception of orthokeratology, w hich accounts for about half of Hong Kong's rigid lens fits. Daily disposables are w idely prescribed in Hong Kong; they account for 39 percent of all fits and 72 percent of new soft daily w ear spheres. Analysis of the remaining lens types show s a significant uptake of silicone hydrogels for daily w ear. Extended w ear of soft lenses remains unusual. Figure 3 compares the Hong Kong market w ith that of the United States.

Daily Disposable Lenses Daily disposable contact lenses continue to be the most enigmatic of lens types, as highlighted by their usage in the tw o biggest markets in the w orld: daily disposable lenses account for 43 percent of new soft lens fits in Japan and 5 percent of fits in the United States. This discrepancy is sometimes explained by the popular use of one- to tw o-w eek lenses in the United States due to a perceived similarity betw een this replacement frequency and daily disposable lenses. How ever, the high number of one- to tw ow eek lenses prescribed in Japan — 46 percent of new soft lens fits compared w ith 55 percent in the United States — undermines this explanation. Figure 2 show s that the proportion of daily disposable contact lens fits has fallen in the United States over the past five years and has remained approximately constant in the four other markets show n. As in previous years, daily disposable lenses are the most successful in Scandinavia, accounting for 63 percent and 57 percent of new soft daily w ear fits in Norw ay and Denmark, respectively. If w e restrict this analysis to spherical lenses only, these values increase to astonishing levels: 84 percent and 81 percent, respectively. Daily disposables are also commonly prescribed in the Middle East countries represented in this survey. The proportion of daily disposable lenses prescribed as new daily w ear fits in Jordan, Kuw ait and the United Arab Emirates varies betw een 23 percent and 62 percent, among the highest of the markets assessed.
