Dear Mr. Bell, I am writing in response to your email dated 07/06/2013 requesting information under the Freedom of Information Act 2000 regarding weekend Treatment of Sub Arachnoid Haemorrage. Q1

Whether your neurosurgery unit undertakes coiling of patients with a sub arachnoid haemorrage out of normal hours ( i.e after 5.30 pm)or at the weekend ?

A1

We do not routinely provide aneurysm coiling after 5:30 pm but weekday cases frequently continue beyond this time due to late start (see below). Occasionally a procedure will be carried out when the on call duty consultant for the regional diagnostic neuroradiology service is also an interventional neuroradiologist able to undertake this procedure. This is the case 3 weekends in 7.

If you do not, is there a system to enable patients to be transferred from your NHS Trust to a hospital that does undertake weekend/ out of hours coiling of a sub arachnoid haemorrage patients ?

This is ultimately a decision for the admitting neurosurgical consultant. There is no formal referral network and there is limited availability of weekend coiling in geographically appropriate Centres. In practise, the combination of Friday pm and Monday am access for emergency coiling and 3/7 interventional neuroradiologist weekend cover means that timely access to treatment is achieved for the large majority of patients in this Centre.

If there is no such system at present, are there plans to create such a system in the future?

We await the recommendations of the current NCEPOD enquiry into SAH management pathway. The possible requirement for increased access to emergency coiling, which might include recommendations on formal Networks is within the scope of this enquiry.
