early 60s Dr. Arnold Kadish of Los Angeles, California, devised the first insulin pump in the early 1960s. It was worn on the back and was roughly the size of a Marine backpack (Figure 1). Rigorous clinical testing for CSII began in the late 1970s,[3-5] and by the early 1980s, CSII was being considered as a possible alternative form of insulin delivery for patients with type 1 diabetes.[6] HR HENRICHS
Slama, Hautecouverture, Assan, Tschobroutsky 1974 • 7 type 1 diabetic patients • Regular insulin IV for 1-5 days from peristaltic pump in shoulder bag • Basal rate and 15 fold higher prandial rate • Prolonged insulin infusions feasible • Very good glycaemic control possible without closing the loop but IV route has problems HR HENRICHS

Continuous subcutaneous insulin infusion versus multiple daily injections: the impact of baseline A1c. Diabetes Care 27 2590-6 2004
importantly, the interaction between baseline A1c and treatment modality emerged as an independent predictor of treatment effect (CSII - MDII) (P = 0.002). The relative benefit of CSII over MDII was found to increase with higher baseline A1c. A model derived from these data predicts that in a patient with a baseline A1c of 10%, CSII would reduce the A1c by an additional 0.65% compared with MDII. Conversely, there would be no A1c benefit of CSII compared with MDII if baseline A1c were 6.5%. HR HENRICHS

CSII 2007 - Conclusion (1) • CSII is available • CSII penetration in Europe is on the way but • very different among countries (all over) • CSII-interest is growing constanty HR HENRICHS
CSII 2007 - Conclusion (2) • CSII is superior to intermittend insulin – to decrease number of (severe) hypoglyc reactions – to lower HbA1c, especially • when initially high • in „hypo-prone“ patients
– to decrease BG-variability • ((CSII may have an impact in chronic complication))
• CSII has reached patients acceptance
HR HENRICHS
CSII 2007 - Conclusion (3) • T1DM-pat, who may/will profit from CSII: – 5% – 9% – 15% – ~5%
severe hypos under ICT/MDI with hypos disrupting daily life having HbA1c>9.0% under ICT/MDI with dawn phenomenon under ICT/MDI
– young children, pregnant women
HR HENRICHS
CSII 2007 - Conclusion (4) • 25-30 y of CSII saw: – technical improvements – increasing penetration into routine – increasing acceptance
– step by step instead of per revolution – further (great) progress is to expect in the future (but don´t expect artificial beta-cells soon)
HR HENRICHS
CSII and IDF-E – a paradigm for • support by information • comparison, which creates questions (and answers) – and may stimulate standards (indications, procedures, care...)
• transfer of knowledge & experience • hopefully concerted actions (training courses, workings- or study groups)
