Dengue epidemics in the South Pacific 1970-1998 vs El Niño

• Enhancing the capacity of the health sector to respond to climate sensitive diseases (CSDs) • On-going assessment of health vulnerability to climate change, to develop early warning systems. • Sensitization of health and other sectors about the relationship between climate change and diseases. • 2011-2012: clarification and harmonization of national mandates and procedures.

Challenge: Encouraging multi-sectoral, transdisciplinary cooperation for assessing health vulnerability to climate change.

APSED/PRIPPP Nauru • H1N1 mass vaccination – Nauru achieved 96% coverage of total population (aim was 90%) – Impact: No new H1N1 cases detected since Oct 2010



Syndromic surveillance – Detected a large diarrheal disease outbreak in early 2011 – Impact: Rapidly controlled through water disinfection

French Polynesia • Syndromic surveillance linking into the Pacific system • Weekly feedback reports • Impact: – System has been quick to detect outbreaks of dengue and influenza – More effective regional watchfulness (e.g. preparedness for Chikungunya from New Caledonia)

Challenge: Technical support and resourcing by regional partners crucial

NCD Wallis & Futuna

• Support from the 2-1-22 programme has made it possible to – support the establishment of primary services for NCD – Assist in introducing NCD prevention – Better patient monitoring is already being achieved and social awareness is growing all the time

Challenge: We would like an assurance that we will be able to continue to benefit from the 2-1-22 Pacific NCD programme ….

HIV/AIDS & STI PNG • The HIV and AIDS Management and Prevention (HAMP) to enforce human rights and enhance the environment is being reviewed in 2011. • Finalization of the GBV policy and has completed the Clinical Protocol on Sexual Assault. Tokelau • Implement a comprehensive approach to STI control through clinical and prevention services • Ensuring reliable data to inform STI programming. Challenge: PNG, due to the laws criminalizing SW and MSM, has found it difficult to reach these vulnerable groups.

Drug Management • Federated States of Micronesia, Fiji, Palau, Solomon Islands, Tuvalu, Tonga, and Vanuatu developed and reviewed a national medicines policy • Medicines procurement and price survey was conducted in 14 countries. • Pharmaceutical inventory management system was assessed and developed in Kiribati, Palau, Tonga, Tuvalu, and Vanuatu. • National procurement and supply systems were assessed in Cook Islands and Papua New Guinea Challenge: Improved availability of essential medicines & efficiency in medicines procurement and supply would require adequate management capacity and resources (funds, facilities, logistics, etc.)

MCH Kiribati • Family planning is the main focus and on top of the agenda in the Ministry of Health working closely with KIFHA • Family planning is now top of the agenda in the Ministries new strategic plan. • Impact: There will be a less demand on health services in the future. Challenge: Limited access to services of skilled birth attendant

HR and PHRHA Samoa • Member of PHRHA • New Act developed and passed establishing a position for a Registrar Health Professions to advice Councils and keep all Professional Registers • Professional Standards developed for Medical Practitioners, Nursing and Midwifery, Dental, Pharmacists and Allied Health Professionals. • Assuring quality practice Challenges: Inequitable distribution; attritions /migration; low academic standards of school graduates; limited budgets for staffing needs

Aid Effectiveness •

In 2010, the Paris Declaration Survey was implemented in seven countries (Cook Islands, Fiji, PNG, Samoa, Solomon Islands, Tonga, Vanuatu)



Establishment of health sector coordination mechanisms such as the SWAp in Samoa and Solomon



Formalized joint partnership arrangement (JPA) between the MoH in Vanuatu and its development partners (AusAID, UNICEF, WHO, UNFPA and SPC) to deliver the Vanuatu Health Strategy 2010-2016.



Fiji, FSM, Kiribati, Samoa, Vanuatu have developed National Health Accounts as a methodology to track health expenditures – to improve transparency in health funding

Coordination Mechanism; country ownership & leadership; NHAs; etc.

Reduce complexity

Challenges: country ownership & leadership capacity limitations; need for economists /accountants in the health sector.

Health System Strengthening and Primary Health Care Vanuatu •

Implementation of the Healthy Islands & PHC Strategy with the roll-out to all 6 provinces:



Expected outputs: PHC implemented in all 6 provinces, strengthening of health systems at national and provincial level, better access to services at all levels.



Expected Impact: Increased coverage of health services and ultimately leading to improved health outcomes (reduced morbidity and mortality)

Challenge: As far as implementing a sound primary health care system, the outstanding challenges I have is manpower..
