HAAD Requirements for Group Medical Policies Dear Valued Customer, Please note that the Health Authority of Abu Dhabi (HAAD) has set guidelines for all medical insurance providers to comply with. As a result, we need to update our criteria for all employees and dependents under our policies that are operating out of Abu Dhabi. The below criteria will result in your Company, the employee or a dependent being deemed (by law) as required, to be covered under HAAD Compliant schemes and for the purposes of this communication are termed “Abu Dhabi Members”:

Your company is licensed & operating in the Emirate of Abu Dhabi If any employee / dependent is: •

Working in the Emirate of Abu Dhabi



Residing in the Emirate of Abu Dhabi



Holding a Residency Visa issued from the Emirate of Abu Dhabi (even if working / residing outside the Emirate of Abu Dhabi)

If any of the above apply, please confirm as such by providing the following documents by email (as softcopies): •

At the policy level: Trade License of the Company.



At Employee/Dependent level (Please ensure the Employees & Dependents details are entered in the updated Electronic Data Interchange form (EDI) - which can be obtained from MetLife Alico Customer Services):

A) Employees: • Passport size photo in JPG format; • Copy of the passport;

New Comer

• Copy of employment visa issued with entry stamp date; • An undertaking letter; and (Form attached) • Emirates ID (If the emirate ID is under process please provide a supporting document e.g. copy of the stamped application form) • Passport size photo in JPG format; • Passport copy of the Insured; • Employment letter that declares an Insured to comply with HAAD Regulations;

Non Abu Dhabi Member moving to Abu Dhabi Member

• Emirates ID (If the emirate ID is under process please provide a supporting document e.g. copy of the stamped application form); • Copy of Employment VISA; and • One of the below documents: ²² Authorized copy of labor contract; ²² Copy of Labor card issued by Department of Labor; or ²² For government or semi-government entities, a certificate issued by the

employer to confirm the employee is part of the organization • Passport size photo in JPG format; • Passport copy of the Insured; • Continuity Certificate; • Employment letter that declares an Insured to comply with HAAD Regulations;

Abu Dhabi Member moving to new employer in Abu Dhabi

• Emirates ID (If the emirate ID is under process please provide a supporting document e.g. copy of the stamped application form); • Copy of Employment VISA; and • One of the below documents: ²² Authorized copy of labor contract; ²² Copy of Labor card issued by Department of Labor; or ²² For government or semi-government entities, a certificate issued by the

Dependents of Non Abu Dhabi Member moving to Abu Dhabi Member

• Copy of Residency visa issued; and • Emirates ID (If the emirate ID is under process please provide a supporting document e.g. copy of the stamped application form) Note: Emirates ID is not mandatory for children under the age of 15 Years, kindly provide card copy if available. • Passport size photo in JPG format; • Copy of the passport;

Dependents of Abu Dhabi Member moving to new employer in Abu Dhabi

• Continuity Certificate; • Copy of Residency visa issued; and • Emirates ID (If the emirate ID is under process please provide a supporting document e.g. copy of the stamped application form) Note: Emirates ID is not mandatory for children under the age of 15 Years, kindly provide card copy if available.

New Born Baby for Abu Dhabi Member

• Passport size photo in JPG format; • Birth certificate (we can accept hospital birth note, to be followed by the Birth Certificate)

To avoid penalties, the above notifications should be completed within. • The first thirty days for New Born Baby.

Note

• The first week for new comer. • The first thirty days for transfers.

Fine Calculation and Appeal Form: •

If an Abu Dhabi Member has violated HAAD Regulations and has not been covered within the required period (i.e. in line with the above time scales), the fines will be applicable as per HAAD Circular No. CEO/254/11 and Circular No. 35. (Both Circulars are attached for your reference).



An Appeal Form can be submitted to HAAD as per the following:

A) Appeal Form for Employees:

The Employer can apply for an appeal directly to HAAD, for all Employees (and Dependents) that

are under their sponsorship, requesting a waiver for the penalty in question, however a nonrefundable deposit of AED 2,000 (along with AED 100 application fee for each family), must accompany the application.

B) Appeal Form for Dependents: • The Abu Dhabi Member must appeal directly to HAAD for Dependent violations • The below provide the necessary requirements: ²² Dependent Appeal Form (attached) ²² Copy of the previous health card if applicable

²² Passport copy with valid Residency visa for each violator ²² Sponsor’s passport copy ²² Sponsor’s phone number ²² Abu Dhabi Member’s phone number ²² An AED 100 fee.

Please note that the fine should be paid to the HAAD Bank Account, per the following details: Bank Name:

Kindly provide us with a copy of the receipt number from the National Bank of Abu Dhabi.

Certificate of Continuity: • Name of Insurer • Health Insurance policy number • Effective Date of Health Insurance policy • Expiry Date of Health Insurance policy • Policy Type (Thiqa, enhanced, basic, emergency…) • Name of insured • Nationality of insured

Certificates of Continuity should include the following

• Sex of insured: male / female • Date of birth of insured • Authorized signatory (insurance company) • Insurance company seal • Certificate issuance date • The following text: “This is to certify that the person/persons insured under the above mentioned health policy, is/are covered as per Law No. 23 of 2005 regarding health insurance in the Emirate of Abu Dhabi and its Regulations. This certificate is valid for 30 days as of the date of issuance.”

To re-confirm, the above requirements are applicable to any Abu Dhabi Member working or residing in the Emirate of Abu Dhabi OR Holding Abu Dhabi Residency Visa.
