Oemrawsingh 3 on behalf of Cardiology, Haga Teaching Hospital. 1 Haga Teaching Hospital, The Hague, Netherlands; 2 Reinier de Graaf Gasthuis, Delft, Netherlands; 3 Medical Center Haaglanden Westeinde Hospital, The Hague, Netherlands Purpose: To compare a 1-stage PCI to a 2-stage PCI strategy for complete revascularization in STEMI patients with Multivessel Disease (MVD). Method: Consecutive patients with STEMI and MVD (n=1119), presenting for primary PCI (PPCI) between January 2006 and January 2010, without shock or previous CABG, were retrospectively included (n= 832); 61 patients had PCI for all significant stenoses (1-stage PCI; 65±15 yrs., 18% women) and 123 patients (60±11 yrs., 23% women) had scheduled 2-stage PCI. Mortality, non-fatal reinfarction, unscheduled revascularization and cerebrovascular events (MACCE) were recorded during follow-up of 3.5±1.5 yrs. Results: Patients in the 1-stage PCI group were older (60±11yrs. vs. 65±15 yrs.; p=0.001), had less 3-vessel disease (52% vs. 33%, p=0.01) and TIMI 23 flow post-procedure (99% vs. 93%; p=0.02). Multivariate analysis for MACCE demonstrated the 1-stage PCI group as an independent predictor (HR 0.22; 95% CI 0.07-0.73, p= 0.01) and revealed an interaction between treatment strategy and gender (p=0.03). Compared to 2-stage PCI, women had a higher MACCE rate with 1-stage PCI (HR 5.43; 95% CI 1.02-28.94, p=0.048), whereas men had a lower MACCE rate with 1-stage PCI (HR 0.18; 95% CI 0.04-0.98, p=0.048). No significant difference was demonstrated between the groups for mortality.
