1.

Scope of emergency response plan

1.1

Introduction/directions to workers These emergency procedures have been developed to provide advice and guidance for emergency situations that may occur at Dunlop Flooring, 183-187 Newton Road Wetherill Park. These emergency procedures are designed to assist all workers in dealing with any emergency. The effectiveness of this manual depends on the willingness of workers at all levels to make themselves aware of the immediate actions they must take in an emergency so that they are capable of acting promptly, calmly and efficiently. This manual has been compiled from potential possible emergencies, historic on-site emergencies as well as incorporating the requirements of our integrated management system (SH&E). When following this plan, the priority is the safety of workers and visitors. Only if safe to do so, should any attempt be made to protect records, property or other assets. Site management are responsible for ensuring that all workers and contractors working on the site are aware of the location of the following:   

Aim The aim of this emergency response plan (ERP) is to minimise the risk and/or reduce the potential loss as a result of an incident that may occur at a Dunlop Flooring Wetherill Park.

1.3

Purpose The primary purpose is to establish and maintain effective processes to effectively respond, report, register and manage preventative actions arising out of emergencies at 183-187 Newton Road, Wetherill Park. The emergency response plan (ERP) will be reviewed at least every 12 months to ensure continued relevance and application.

1.4

Authority PB has assigned designated workers responsibility as the emergency response team (ERT). All workers who act in such roles have full authority to implement provisions of the ERP and shall be indemnified against liability resulting from training or emergency evacuation, where those workers acted in good faith and in the course of their duties.

A written document (this document) which documents emergency arrangements for our PB sites. It consists of the preparedness, prevention and response activities and includes the agreed emergency roles, responsibilities, strategies, systems and arrangements

Tornado watch

When a tornado watch has been issued in an area, workers should monitor local radio or television for additional information.

Tornado warning

A tornado warning means that a tornado has been sighted by the public, law enforcement or radar has indicated an area of rotation that could develop, or has developed, into a tornado.

Tornadoes

The rarest and most violent of thunderstorm phenomena. They are rapidly rotating columns of air that descend in the well-known funnel shape from the base of a storm cloud.

Visitors and contractors The procedures detailed in this plan apply to all visitors and contractors and should be outlined during induction.

Types of emergencies A building emergency can develop from a number of man-made or natural causes and a full or partial evacuation of the building may be necessary. Refer to the emergency response risk assessment (template available) for emergencies that could eventuate.

2.

Responsibilities

2.1

General All workers have responsibilities to ensure emergencies are managed. Where it is safe to do so, the following responsibilities should be observed during any emergency:

4.

Emergency situations

4.1

Industrial/transport accident or medical emergency

In the event of an industrial/transport accident or medical emergency the following procedure should be initiated: Chief warden (or their delegate)      

Contact PCBU/AM and inform them of the incident Contact the closest first aider The chief warden will advise of a stop work order or site evacuation If required, contact emergency services. You must advise emergency services if a serious injury has occurred The chief warden will broadcast the all clear after the emergency is deemed safe Chief warden to initiate a debrief with the PCBU/AM and ERT

PCBU/AM       

PCBU/AM will designate worker(s) to meet emergency services and direct them to the area If a worker has been injured, arrange for contact to be made with the worker‟s next of kin If a serious incident, where a workers (or other persons) were removed by ambulance, contact the national SH&E manager to discuss the potential reporting requirements to the relevant authority (eg. WorkSafe, WorkCover etc) Incident logged in incident/injury database Conduct full investigation as per the PB investigation procedure Follow all processes outlined for incident response (eg. Injurypak) Conduct rehabilitation (if required)

First aider    4.2

The first aider will assess the nature and extent of the injuries to determine the appropriate process to follow in relation to the medical emergency and will contact emergency services (eg. ambulance) Follow instructions provided by the emergency services operator and stay with the patient until emergency services arrive Do not move the patient unless they are in a life-threatening situation

Fire/emergency In the event of a fire, workers and visitors are our primary responsibility. We do not expect workers to fight fires if they are not comfortable (nor trained) to do so. Buildings and product can be replaced – people can‟t. In the event of a fire/general emergency the following procedure should be initiated: Worker/other person 

Chief warden and ERT leader to assess the situation and implement appropriate action. ERT leader briefs ERT team and initiate response. Chief warden calls the fire brigade or other emergency services (as required). Chief warden to provide:  provide details of the emergency  give address of the site  give details of the area of emergency  explain steps undertaken so far  provide their name and contact number Wait for emergency services to arrive and follow their instructions When the fire has been extinguished and emergency services have given back control to the chief warden, the chief warden will broadcast the all clear to return to the premises Chief warden to initiate a debrief with the PCBU/AM and ERT

Area warden (or their delegate)  

Conduct area search and ensure all occupants have left the building Area warden will collect sign-in records and conduct roll call

Incident logged in incident/injury database Conduct full investigation as per the PB investigation procedure Liaise with the GM SH&E/Risk or national SH&E manager as there may be a requirement to notify the insurers and/or a regulatory body

Gas leak In the event of a gas leak, and if safe to do so, the following procedure should be initiated: Worker/other person 

Worker/other person raise the alarm and evacuate. Worker to inform chief or area warden on the incident.

Chief warden (or their delegate)     

Stops all plant running immediately Advises all workers and sounds the alarm If safe to do so, the ERT team will isolate the gas supply at the source Chief warden calls the fire brigade or other emergency services (as required) and informs neighbours (as required). Chief warden to provide:  address, nearest intersection and best entrance address  what has happened  the type of gas that is leaking (eg. chemical name, refer to the safety data sheet [SDS])  any other hazards emergency services may encounter

Area (or other) wardens        

Turn off the air conditioning and fans Remove all ignition sources (ie. turn off the electricity at the main switch) Assist with evacuation of the building to the emergency assembly point, upwind of the leak Check all areas have been cleared and inform the chief warden Area warden will collect sign-in records and conduct roll call Maintain control of the emergency assembly point Conduct full investigation as per the PB investigation procedure PCBU/AM (or their delegate) will liaise with the GM SH&E/Risk or national SH&E manager as there may be a requirement to notify the insurers and/or a regulatory body

Contact with sprinkler head In the event of contact with a sprinkler head the following procedure should be initiated: PCBU/AM (or their delegate)        

Isolate the area of instability and, if necessary, evacuate workers from the area Contact emergency services and notify them of the situation – follow instructions from emergency services. If directed, turn off the pump (or main) and, if necessary, the electricity to the affected areas Call „Superior Fire Services‟ 02 9986 2233 to undertake repairs to sprinkler head Initiate clean-up of area and resume normal use when repairs complete Incident logged in incident/injury database Conduct full investigation as per the PB investigation procedure Liaise with the GM SH&E/Risk or national SH&E manager as there may be a requirement to notify the insurers of the damage Remove isolation and resume use once the racking is repaired and investigation complete

Structural instability In the event of structural instability the following procedure should be initiated: Racking instability

Isolate the area of instability and, if necessary, evacuate workers from the area Remove stock/assets from racking (only if safe to do so) (as directed by the PCBU/AM or their delegate) Call „Active Storage‟ 02 9526 8422 to undertake repairs to racking Incident logged in incident/injury database Conduct full investigation as per the PB investigation procedure Liaise with the GM SH&E/Risk or national SH&E manager as there may be a requirement to notify the insurers and/or a regulatory body Remove isolation and resume use once the racking is repaired and investigation complete

Other structural instability (ie. property damage) PCBU/AM (or their delegate)        4.6

Isolate the area of instability. Preserve the scene until given the all clear by the national SH&E team Remove stock/assets (only if safe to do so) (as directed by the PCBU/AM or their delegate) Contact the landlord or national property team to repair the unstable structure Incident logged in incident/injury database Conduct full investigation as per the PB investigation procedure Liaise with the GM SH&E/Risk or national SH&E manager as there may be a requirement to notify the insurers and/or a regulatory body Remove isolation and resume use once the structure is repaired and investigation complete

Building invasion (intruders) In the event of building invasion / disturbance the following procedure should be initiated: Workers         

At all times keep cash, vital records, information and valuables secured and to a minimum Do not discuss activities, vital records, amounts of cash or security procedures in public Notify your supervisor/manager if you are suspicious of the behaviour of any persons Try to remain (or appear) calm Do not make any sudden movement or take any action to excite or aggravate the intruder(s) Be courteous, converse with and answer questions by the intruder(s) Obey instructions given by the intruder(s) If requested, immediately handover valuables (eg. cash, product etc) – never challenge or question intruders Be observant to enable completion of the armed intrusion checklist after the event

4.7

Neighbouring property incident In the event of an emergency (eg. fire) occurring at a neighbouring property, the business is to undertake the following procedure: Chief warden (or their delegate)  

Chief warden to provide:  address, nearest intersection and best entrance address  what has happened  give details of the area of emergency  any other hazards emergency services may encounter  their name and phone number Follow all directions provided by the fire brigade or other emergency services Chief warden or delegate to confirm the neighbouring property is aware of the incident Chief warden to prepare the site for evacuation (if necessary) Initiate head count of workers and make telephone contact with those who are not on-site Chief warden to initiate a debrief with the PCBU/AM and ERT

Terrorism Given the nature, location and operations of the business, the impact or potential impact of terrorism in a PB business is seen as a low risk occurrence. In the event that an act of terrorism may occur, workers are to follow the procedures provided under building invasion.

General control in place consist chemical shower, Decontaminant, PPE, SDS, trained ERT, Certified and trained chemical handler. Copy of SDS summary supplied as appendix 18 for above chemicals, full SDS available on site. In the event of a hazardous substances or dangerous goods incident the following procedure should be initiated: Spill or substance exposure Chief warden/ERT Leader     







Isolate the area the spill has occurred in. If the substance cannot be identified, do not make any attempt to clean up the substance Protect / cover any drains in close proximity to the spill Immediately retrieve a copy of the safety data sheet (SDS) if substance is known Large spill If the spill is too large for immediate clean up, isolate the area and contact emergency services Spill emitting toxic fumes If the spill is emitting toxic fumes:  turn off air conditioning and recirculation fans  turn off all effected equipment  open doors to ventilate air (if possible)  evacuate premises upwind of the spill  retrieve the TDI measuring monitor and measure the TDI levels (if applicable) Retrieve the site spill kit and:  apply absorbent material to the spill area (wearing the correct personal protective equipment)  use the specific broom to move the absorbent material around the spill area until all the liquid has been collected  when the absorbent material is ready for removal, use an appropriate tool (eg. spade) to gather the material and place in secure bags  when clean-up is complete, contact a licensed prescribed waste disposal company for collection of the material If applicable, evacuate the area, including all persons who may have been injured or suffered exposure to the substance involved in the incident. Ensure emergency assembly point is at least 200m clear of the spill. This may require a temporary change to the emergency evacuation assembly areas/points. If applicable, inform neighbouring properties about the incident and request them to evacuate if required.

emergency services (eg. ambulance) Follow instructions provided by the emergency services operator and stay with the patient until emergency services arrive Do not move the patient unless they are in a life-threatening situation

Fire water runoff In the event of a site experiencing a fire emergency, it is recognised that the runoff from the water used to extinguish the fire may be contaminated and pose a hazard to the environment. Where possible, and only if safe to do so, the emergency response team is to cover all stormwater drains for containment. Where it is not possible or not safe to do so, firewater management is to be at the discretion of the emergency services. The business will contribute to this in line with any requirements set out by the Environmental Protection Authority (EPA). 4.11

Severe weather including storm, cyclone, flood, earthquake etc In the event of a severe weather incident, the following procedure should be initiated: Chief warden (or their delegate)       

  

 

Activate the ERP and ERT Stop all plant running before the severe weather hits (or as soon as it hits) If possible, store/secure loose items external to the buildings Secure all doors and windows If time permits, tape glass windows and doors and, if necessary, protect them with timber and/or sand bags If time permits, protect valuables, cover or move away from windows Chief warden calls the fire brigade or other emergency services (as required). Chief warden to provide:  address, nearest intersection and best entrance address  what has happened  how many people are injured  any hazards emergency services may encounter  their name and phone number Chief warden to follow instructions provided by the operator. Chief warden may be directed to isolate and shut off electricity, water and gas services Chief warden to evaluate the need to evacuate if fire, gas leaks or structural damage occurs during the storm With PCBU/AM input and if the storm is deemed severe enough, the chief warden should consider:  sending workers home before the weather hits the site (if there is any uncertainty workers should remain in a building, keeping away from windows, until the storm passes)  in advance, calling workers and advising them not to come into work until the weather has moved away from the site  stopping work and evacuating workers to a secure area off the site (eg. office)  isolating vulnerable areas of the site until the weather has moved away from the site Post-storm, the ERT should assess the damage and report it through to management Chief warden to initiate a debrief with the PCBU/AM and ERT

If a tornado watch has been issued, monitor radio stations for updates The chief warden (or their delegate ) must notify all workers in the building of the tornado watch. This notification must include:  building is under tornado watch  time the tornado watch expires  normal routine will not be interrupted unless a tornado warning is issued If a tornado warning has been issued, all workers must proceed to the nearest designated shelter or safest area in the building (away from buildings, glass windows and away from items that may collapse). Suggested safe areas are:  interior rooms and passage ways in large buildings and / or the lowest floor  central stairwells  basements (if applicable) Chief warden to direct workers to safe areas (like under solid furniture), crouch down and cover their heads Post-tornado the ERT should assess the damage and report it through to management Chief warden to initiate a debrief with the PCBU/AM and ERT

Bomb threat procedure (including letter bomb) What to do (a)

Written threat If a bomb threat is received in writing, it should be kept, including any envelope or container. Once a message is recognised as a bomb threat, further unnecessary handling should be avoided. Every possible effort has to be made to retain evidence such as possible fingerprints, handwriting or typewriting, paper and postmarks. Such evidence should be protected by placing the evidence in a plastic sleeve or envelope (plastic envelope is the preferred option).

(b)

Telephone threat An accurate analysis of the telephone threat can provide valuable information on which to base recommendations, action and subsequent investigation. The person receiving the bomb threat by telephone should not hang up and, as soon as possible, complete the information required by a bomb threat checklist.

(c)

Suspect object A suspect object is any object found on the premises and deemed a possible threat by virtue of its characteristics, location and circumstances.

Ensure that the bomb threat checklist (attached to this procedure) is completed to the best of your capabilities, as this will greatly help the appropriate authorities. Who to inform Upon receipt of a bomb threat do not hang up the phone the call was received on. Immediately notify the chief warden and the PCBU/AM will contact the police and inform them of the situation. That call should be made from another phone so that we do not hang up the line the threat was made on. After the call It is PB policy, as set out in the emergency manual, for the entire site to be evacuated. However, prior to this the ERT should take steps to ensure that the evacuation areas are safe and that there is no danger in evacuating workers and others. Once this has occurred the evacuation alarm should be sounded and the normal evacuation processes put into place. ERT team undertakes internal briefings to ensure all staff are familiar on regular basis

Fire prevention and control methods In order to control the risk of a fire, several measures have been taken. These include:  

all PB sites have a no smoking policy within buildings and under cover areas. In all other outside locations, there is a restriction on smoking. Smoking is only permitted within designated smoking areas the enforcement of hot works permits. No hot work is to take place without first completing a hot work permit and following the hot work procedure. The hot work procedure requires a fire team member to be present with suppression devices in the event of a fire breaking out scheduled electrical inspections of all machinery and wiring throughout the site the provision of portable fire fighting equipment in line with the Building Code of Australia and the relevant state building code. Inspection of these devices is carried out in accordance with the appropriate regulations the business will undertake evacuation exercises at least every 12 months to test the effectiveness of the ERP.

After the event (debrief) Following a critical incident, a debriefing is required to confirm how the ERP/ERT performed, if counselling of workers is required as well as the necessary communication being delivered through to the CEO. All counselling is to be organised with the assistance of Human Resources. A critical event is often traumatic to those involved and therefore counselling is recommended. After recovery from the event, the debriefing should involve the following:      
