•According to the famous physicist and Nobel Prize winner, Albert Einstein, “Science without

religion is lame. Religion without science is blind.”

•The Quran is not a book of science but a book of ‘signs’, i.e. ayats. There are more than six

thousand ‘signs’ in the Quran of which many deal with science. Given are 2 major examples:

•My first work was started from the ultrasound clinic of UOL old campus. •On the topic “DOPPLER ULTRASOUND IN

•At 10/10/2013 our thesis defense was conducted at UOL old campus. Among other students I was also succeeded.

•Any by the grace of Almighty Allah I was awarded with Gold Medal in Medical Ultrasound.

Effort to publish in the journal of AIUM

I sent my thesis to the editor in chief to publish it. It was comprising about 127 pages. The Editor sent me the journal guidelines and advised me to prepare it according the journal format.

It was difficult for me to even read the journal guidelines. I merely eliminated extra litters and sent again.

This time the journal editor advised me to take advantage from any researcher in my country. Because I don’t know even the A, B, C or research and publication.

Disheartened

Hope •I was collecting data from patient history in the note

book and sonographic data i.e. findings in the machine. •With a hope that one day I’ll got free and learn to publish.

Lecture of biostatistics •I delivered my first official lecture on biostatistics to the students of MS-Ultrasound.

Hope is near to be proved true •Once again I prepared my article on the first thesis, when publication became hurdle in the way of my promotion. •This time Dr. Murtaza MLT Department helped me too much.

Patience at the time of publication •I prepared my article according to

•Then sent to IMJ (Isra Medical Journal). Because all the medical journals should obey the ICMJE rules

IMJ return my article for gross changes

•Those gross changes were; to make the article according to the journal style which is given in the journal guidelines.

•Now I realized that I have wasted my time in hurry (Hurry makes worry).

Spontaneous nephrocutanous fistula •I searched for that case in my record book but too difficult. •I got help of my ARDMS student (Iqra Manzoor) and finally find it. •I made this case report according to AIUM guidelines. It was totally new and interesting case for me and I was sure that AIUM will publish it.

Abstract •Introduction: Spontaneous nephrocutaneous fistula is a rare complication of renal diseases such as renal calculus, chronic pyelonephritis, stricture of the ureteropelvic junction, renal tuberculosis (TB) and chronic pyelonephritis. Sometimes it can also be expressed as a complication of renal rupture, surgery, renal mass or any other renal interventional procedure.

Abstract •Case report: We reported a case of 30 years old women having spontaneous nephrocutaneous fistula, with ultrasound. Patient came

with the complaint of purulent secretion from the right subcostal margins. She had no history of pain, diabetes, hypertension or any other surgical or interventional procedure. According to the patient history she got pain in right lumber region about ten years back. She underwent IVP but she was diagnosed for right renal stones and renal shout down.

Abstract •Material and method: we examined the patient with ultrasound in supine, right and left decubitus positions, for abdominal and superficial structures according to

the AIUM guidelines. Toshiba xario having Convex and linear transducers with color, power and spectral

Doppler were used for this examination.

Abstract •Case Sonographic findings: We observed multiple stones in the right kidney

with parenchymal rupture. The ruptured site of the kidney contained a stone appearing to have sharp projection and was thought to be the cause of rupture. By following the cutaneous ulcer a communication (fistula) was found with the kidney at rupture site. Now moment of the kidney was detected with respiration, seemingly to be connected with the surrounding perinephric structures. The ruptured show resistive flow but blood flow was present on Doppler examination. Adequate amount of functional renal parenchyma was present. Latter on the Labe analysis of the secretion shows urine contents (asked by Phone)

Abstract

•Conclusions: Chronic renal stones could be the cause

of

renal

rupture

and

spontaneous

nephrocutaneous fistula. Ultrasound is the first line modality to diagnose it with high accuracy.
