 Complete and submit various health forms

• Send Proof of Insurance, Student Health Information Questionnaire, and Tuberculosis Risk Assessment forms to the Admission Office • Forms can be printed from www.hendrix.edu/Admission under Accepted Students tab • Due by August 21

 Watch for health insurance information

• Watch for a mailing regarding health insurance requirements, waiver, and Web site contact information from the Health Center • Direct all questions to the Health Center at (501) 450-1448 • Due by August 1

 Sign-up for an Orientation trip

• Brochures will be mailed and a Web page launched in May • Registration may be completed online at www.hendrix.edu/Orientation or via the brochure reply card

 Check out the New Student Web page (www.hendrix.edu/newstudents) • Look for new information and updates

 Complete the New Student Planning Assessment

• Information will be sent with the Academic Welcome mailing in early June, which will be followed by the Assessment Mailing • Due by August 2

 Sign Master Promissory Note

• A letter will be sent out from the Office of Financial Aid around May 11, which will include instructions about this

 Verify financing options

• Fall 2007 Hendrix statements of account will be mailed soon after July 4 with payments due in mid-August • Make sure your check is mailed or financing options (Voyager Fund loan, Parent Loan for Undergraduate Students, Tuition Management Systems 10-month payment plan) are in place • Financing option details can be found online at www.hendrix.edu under the financial aid screens or by contacting the business office or the financial aid office • Payments due by August 10
